Release Agreement: I hereby acknowledge that I am voluntarily participating in skating activities organized by Kelly Shock. In consideration for being allowed to participate in these activities, I agree to the following terms and conditions:
Assumption of Risks: I understand that participating in skating activities involves inherent risks, including but not limited to falls, collisions, and injuries. I voluntarily assume all risks associated with participating in these activities, including any risks that may arise from the actions or negligence of other participants, coaches, instructors, or Kelly Shock herself.
Release and Waiver of Liability: I release and discharge Kelly Shock, and any employees, volunteers, coaches, instructors, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may occur as a result of my participation in the skating activities, regardless of whether such liability arises from negligence or any other cause.
Indemnification: I agree to indemnify and hold harmless Kelly Shock, employees, volunteers, coaches, instructors, and agents from any and all claims, damages, liabilities, costs, or expenses, including legal fees, arising out of or related to my participation in the skating activities.
Medical Treatment Authorization: In the event of an injury or medical emergency, I authorize Kelly Shock to obtain necessary medical treatment and/or transportation on my behalf or the behalf of my child. I understand that I am solely responsible for any costs associated with such treatment or transportation.
Photographic and Video Release: I grant RCSA the irrevocable right to use any photographs, videos, or other media taken during the skating activities for promotional, educational, or informational purposes without compensation or further consent unless otherwise indicated as per the RCSA Photo Release Option. Kelly Shock is focused on ensuring the wellbeing and safety of all skaters, coaches, volunteers, staff, and all other participants. Kelly Shock recognizes the potential severity of a head injury and the commitment and intent behind research to manage concussions. Kelly Shock is committed to; educating those involved, creating awareness to help prevent injuries and appropriately managing any suspected and diagnosed concussions.
Definitions: Concussion: A brain injury that causes
➔ changes in how the brain functions, leading to symptoms that can be physical (e.g., headache, dizziness), cognitive (e.g., difficulty concentrating or remembering), emotional/behavioral (e.g., depression, irritability) and/or related to sleep (e.g., drowsiness, difficulty falling asleep).
➔ May be caused either by a direct blow to the head, face or neck, or a blow to the body that transmits a force to the head that causes the brain to move rapidly within the skull
➔ Can occur even if there has been no loss of consciousness (in fact most concussions occur without a loss of consciousness)
➔ Cannot normally be seen on X-rays, standard CT scans or MRIs
➔ Can result in symptoms that are evident immediately, or may evolve and persist over the course of hours, days or even months.
